First Visit

Most patients are referred to us by their Primary Care provider.  In some circumstances patients may be referred by another medical or surgical subspecialist in the community.  Your referring provider will arrange for all pertinent medical records including labs and x-ray results to be sent to our office. Upon receipt of those records our scheduling coordinator will contact you to arrange an appointment with one of our providers.

Any records of previous treatment by another Rheumatologist should be provided to us for review.

When you arrive for your initial visit please be sure to bring:

  1. All completed forms (we will mail these to you prior to your appointment or you may print them from this site and complete).
  2. Photo ID
  3. ALL insurance cards and prescription drug cards. Insurance referrals (as required by your insurance plan) must be presented at check-in.
  4. A complete list of all prescription and over-the-counter medications (including vitamins, supplements, etc.) complete with dosage and strength.


Co-Pays and Insurance:

Co-payments and/or co-insurance are required at the time of service.  For patients without insurance, with coverage by a non-participating insurance plan, or with a high deductible insurance plan payment in full will be required at the time of service.  Our billing specialist or the office manager can assist you with determining that amount prior to your visit so that you can be fully prepared to make that payment.
